Please have your healthcare provider fill out the Medication Authorization form if ANY OVER THE COUNTER or PRESCRIPTION MEDICATION will be needed at school. Forms must be signed by a PARENT/GUARDIAN AND PROVIDER.

 

Please have your healthcare provider fill out a Healthcare Action Plan if needed. (Needed for Asthma, Anaphylaxis, Migraines, Seizure, etc)

 

IMMUNIZATION EXEMPTIONS: Previous Non-Medical Exemptions need to be renewed every school year after June 30th. 

INCOMING 6TH GRADE: Tdap is a required immunization upon entry to 6th grade. Please check with your healthcare provider before the start of the school year to set up an appointment. The Colorado Immunization deadline is November 1st of the current school year.
