School Required Immunizations

  • Hepatitis B (3 doses)
  • Diphtheria, tetanus, pertussis (5 doses)
  • Inactivated poliovirus (4 doses)
  • Measles, mumps, rubella (3 doses)
  • Varicella (2 doses)
  • Tetanus, diphtheria, pertussis (Tdap)

**Students are required to have Tdap prior to entry into 6th grade. One dose of Tdap is required for 6th through 12th grades.

**INCOMING 6TH GRADE: Tdap is a required immunization upon entry to 6th grade. Please check with your healthcare provider before the start of the school year to set up an appointment. The Colorado Immunization deadline is November 1st of the current school year.

 

Medical and Non-Medical Immunization Exemptions

Non-Medical exemption must be completed yearly after June 30th for the following school year.
